Open any audio file, and then click-and-drag to create a region for your marker and associated comments.
Click and drag on a waveform to create a marker region.
When you create a marker, you'll notice the Details sidebar appears on the right side of your screen. Here, you can add a note to that comment, and have an entire chain of comments and replies stored along with that original marker.
